Billing Services

Witham Patient Accounts and Witham Physician Billing are now located in Witham's South Pavilion on the Witham Campus. The address is 2505 N. Lebanon Street, Suite 220, Lebanon In 46052. All phone numbers remain the same.

When you or a family member needs medical care, you should be able to focus on health needs above all else. We understand your concerns, and we try to reduce your stress by simplifying the medical billing process. 

Online 
Our online bill pay allows you to pay your bill online, free of charge. This secure service gives you the ability to pay your bill from your home computer. Click here to pay your bill online.

Pay by Phone

Witham Hospital: 765-485-8060 a customer service representative can accept payment by credit card over the phone Monday - Friday 8:00 am - 4:00 pm. 

Witham Physician Billing: 765-485-8677 a customer service representative can accept payment by credit card over the phone Monday - Friday 8:00 am - 4:00 pm. 

Witham Convenient Care of Lebanon: 765-335-0123 a customer service representative can accept payment by credit card over the phone Monday - Friday 8:00 am - 8:00 pm and 9:00 am - 2:00 pm Saturday and Sunday.

Pay by Mail 
You can pay your bill through the mail using a check, money order or major credit card. When mailing a payment, please include the statement voucher connected to the billing statement.

  • Credit Card (Discover, MasterCard, or Visa)
  • Check

You can also use these forms of payment:

  • Cashier’s Check
  • Money Order
  • Cash (in person only, please do not mail in cash payments!)

If you are unable to pay your bill in full, Witham Health Services offers payment plan options and has a financial assistance program, which you may qualify for. If you need to file for financial assistance, please complete the Financial Assistance Application.  Witham is pleased to recognize our billing staff as Certified Patient Account Technicians through American Association of Healthcare Administrative Management. We hope you will always feel confident in Witham Health Service's commitment to your health.
