View your upcoming hospital and physician office appointments.

Review your medical information, including medications, allergies, and more. Keeping your record up-to-date helps you and your care providers.
Review Test Results
View your lab and radiology test results. Lab results will be released immediately, radiology results will be available after confirmed by a provider.
Pay Your Bill Online
This feature allows you to pay your Witham bills.
Update Your Personal Information
Did you move or change your phone number? Update your personal information on the Patient Portal, which will also save you time during your next visit.

MHealth app is available on Apple and Google Store and provides you access to the Portal on your phone.
Frequently Asked Questions
The Witham Patient Portal connects you with your health information 24 hours a day. You can sign up and register for the Patient Portal by providing your name and email address. In turn, we will send you an email with your temporary username, password and simple instructions. You will then be allowed to log into the system and create your private user name and password.
Your one-time password will expire in 14 days if not activated. To request a new password, call 765-485-8576 and you will be issued a new temporary password by email to the e-mail address we have for you on file.
Portal support requests are handled Monday through Friday, between 8 a.m. and 7 p.m. Be sure to leave a daytime phone number in your request.
If we have a signed release on file, you can be added as a proxy to manage their care on the patient portal. You can also send an email invitation for others to access your account.
Yes. Portal passwords are encrypted. You and only the family members you authorize can access your information.
