Transitions Geriatric Psychiatry
Each stage of life brings with it unique health needs. That is especially true for older adults. We understand the health care concerns faced by many seniors today and are committed to helping you manage your health so you live this stage of life as fully as possible. We offer relevant inpatient and outpatient programs and services that address the health needs, wellness and emotional well-being of seniors.
Behavioral Health
Witham is committed to serving the behavioral health needs of older adults. We offer Transitions, an effective inpatient program that addresses the emotional, as well as medical needs of senior adults so they may maintain a satisfying quality of life. The program offers a safe, secure environment to adults ages 60 and over.
Patients may be referred to Transitions by physicians, social service agencies, family members or other concerned individuals 24-hours a day.
Transitions services include:
- Crisis stabilization
- Individual and group therapy
- Patient and family education
- Medication management
- Activity therapy
- Physical therapy and other ancillary services, as appropriate
- Multidisciplinary assessment
The Care Team Transitions offers a specially-trained, interdisciplinary staff experienced in working with the unique needs and concerns of senior adults. Our team includes psychiatrists, primary care physicians, nurses, social workers, and occupational and recreational therapists. The patient's family is a very important part of the treatment team. Family members are encouraged to participate in the patient's care and discharge plans and family conferences.
Individuals who may benefit from treatment commonly display a number of warning signs, including:
- Withdrawal from social contact
- Loss of interest in normal activities and appearance
- Panic attacks
- Severe change in eating and/or sleeping habits
- Sudden change in mental status
- Suicide threats and/or attempts
- Sudden agitation or aggression
